About The Position

Trinity Health at Home is hiring a Registered Nurse to provide compassionate, high-quality home-based care in the Ann Arbor and Ypsilanti coverage regions. The RN provides primary nursing care as ordered by physicians, develops individualized care plans in collaboration with the Population Health Case Manager, and supports positive patient outcomes and satisfaction. The position includes a $15,000 sign-on bonus.

Requirements

• Graduate of an accredited nursing program; a BSN is preferred.
• Current RN licensure in the state of practice.
• At least one year of professional nursing experience.
• Strong computer and technology skills.
• Current driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments at admission and at ongoing intervals.
• Develop and implement individualized, assessment-based care plans in partnership with physicians and case managers.
• Provide therapeutic interventions and education to promote patient independence and improve health outcomes.
• Monitor and report changes in patient condition and adjust care plans as needed.
• Use motivational interviewing and health coaching techniques to support patient goals.
• Document care accurately and promptly in accordance with agency policies and regulatory standards.
• Mentor and support team members to ensure quality care delivery.
