About The Position
The professional nurse provides patient- and family-centered care using the nursing process and addressing each patient’s physical, psychological, emotional, and social needs. The nurse develops individualized plans of care with patients and their designated care partners, taking into account their priorities, cultural beliefs, and values. The role applies evidence-based practice and clinical expertise, supports patient education, follows nursing practices and protocols to minimize risk, and uses information technology and data to support safe, ethical, and continuously improving care. The nurse works effectively with nursing and interprofessional teams to provide compassionate, coordinated care.
Responsibilities
- Develop individualized nursing plans of care with patients and their designated care partners.
- Integrate evidence-based practice, clinical expertise, and patient and family preferences and cultural beliefs into healthcare delivery.
- Identify patient and care-partner learning needs and priorities, and provide useful, understandable, and affirming education.
- Follow established nursing practices and protocols and individual performance expectations to minimize risk of harm.
- Use information technology to communicate, manage knowledge, mitigate error, and support ethical decision-making.
- Monitor care-process outcomes using data and apply improvement methods to test changes that improve patient-care quality and safety.
- Work effectively within nursing and interprofessional teams, fostering open communication, mutual respect, and shared decision-making.
- Provide safe, compassionate, and coordinated care that respects patients’ personal preferences, priorities, needs, and cultural beliefs and values.
- Interact with co-workers, patients, families, designated care partners, and other staff in a manner consistent with Jefferson’s values.
