About The Position
Energy Employee Services, LLC is seeking a compassionate Registered Nurse Case Manager to join its home health team. The role manages home care for terminally ill or elderly patients, with a focus on accessible, comprehensive care for patients and their families. The position is full-time in Cincinnati, Ohio, with a weekday day-shift schedule and participation in an assigned on-call rotation. The listed pay is $45–$48.00; no pay period is specified. The nurse assesses patients in their homes, oversees physician-provided plans of care, mentors field staff, coordinates with staff and patients’ providers, provides medication education and management, assists with activities of daily living, and documents patient status and condition accurately and promptly. The role requires travel to patients’ homes, potentially within a 45-mile radius.
Requirements
- Be willing to submit to a background check.
- Have a valid driver’s license, reliable transportation, and auto insurance.
- Hold an unencumbered RN license in the state where applying.

- Hold an active CPR certification.
- Have strong communication skills and a committed dedication to providing patients and families with accessible, comprehensive care.
Responsibilities
- Assess patients in their homes to ensure appropriate patient care.
- Oversee each patient's physician-provided Plan of Care (POC).
- Guide and mentor field staff to ensure the Plan of Care is followed accurately.
- Work with staff and patients’ providers to support the best patient outcomes.
- Provide medication education and medication management.
- Assist with Activities of Daily Living (ADLs) to support patients’ quality of life.
- Participate in the on-call rotation as assigned.
- Document patient status and condition accurately and promptly.
- Complete other tasks as assigned.
