About The Position

This is a travel-temporary assignment for a Registered Nurse in Case Management in Roseville, California. Requirements

• Active RN license and current licensure as required by state regulations
• Ability to operate in a care management and care coordination environment
• Strong communication and collaboration skills with interdisciplinary teams
• Competence in care planning, utilization review, and documentation
• Availability to work 8-hour shifts, five days per week

Responsibilities

• Manage the development, implementation and evaluation of the care management service lines for health plan members.
• Promote continuity of care and cost effectiveness through whole-person in-patient and out-patient management, care integration, case management, utilization review and care planning.
• Coordinate with interdisciplinary teams to support member outcomes, transitions of care, and appropriate resource use.
• Document care plans and maintain accurate records to inform care decisions and quality improvement initiatives.
