About The Position

Elara Caring is seeking a compassionate, committed Private Duty Licensed Practical Nurse to provide care in patients’ homes. The role involves reviewing care plans, physician orders, and clinical information before providing care; assessing patient needs; monitoring for changes in condition; communicating concerns to physicians and the care team; and collaborating with the RN Case Manager and clinical leadership on interventions. The nurse maintains timely, accurate documentation and adapts to changing patient care needs, schedules, and assignments.

Requirements

• Licensed Practical Nurse (LPN) in the state or in accordance with Board of Nurse Examiners rules for the Nurse Licensure Compact (NLC).
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• At least 1 year of experience as a nurse in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Review patient care plans, physician orders, and clinical information before providing care.
• Use clinical assessment skills to identify patient needs and make appropriate care decisions.
• Monitor patients for changes in condition and promptly communicate concerns to the physician and care team.
• Collaborate with the RN Case Manager and clinical leadership to determine appropriate interventions when patient needs change.
• Maintain accurate and timely documentation of care provided, patient condition, and changes in the home or caregiver situation.
• Maintain flexibility with scheduling and patient assignments to support changing patient care needs.
