Placement Coordinator PRN Days
About The Position
The Placement Coordinator works under the direction of the RN Case Manager or Social Worker to assist with post-hospital services and coordinate post-hospital care and service arrangements. As part of the Center for Operational Resource Efficiency (CORE), the role works with patient care areas, support services, administration, and others to support seamless patient care, capacity management, efficient patient flow, and improved access and quality of care. This includes reducing missed transfer opportunities, increasing targeted volumes, reducing unnecessary length of stay, and helping place each patient in the right setting at the right time. The position coordinates identified services and helps remove barriers so patients can transition through the care continuum and discharge promptly.
Requirements
- High school diploma required.
- Computer literacy required.
- Strong knowledge of medical terminology required.
Nice To Haves
- Two years of experience in a health care setting preferred.
Responsibilities
- Interact with co-workers, visitors, and other staff in a manner consistent with Jefferson's values.
- Provide prompt feedback to Case Managers and the Physician Advisor regarding payer determinations for post-acute service denials.
- Obtain payer authorization and coordinate identified home care, Durable Medical Equipment (DME), transportation, infusion services, and post-acute services for identified patients.
- Facilitate and eliminate identified barriers to expedite patient discharges.
- Collaborate with the Inpatient Case Manager and others to remove barriers and transition patients through the care continuum.
