About The Position

Signify Health, part of CVS Health, is seeking a part-time Physician to provide in-home health evaluations in Portland and the surrounding area. Physicians engage with people in their homes, build trusted relationships, help bridge gaps in care, and support improved health outcomes through flexible, comprehensive, and personalized evaluations.

Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences, where conditions may include household pets, secondhand smoke, and other airborne allergens or irritants.

Requirements

• Active, unrestricted medical license(s) in the coverage area(s).
• Able to cover Portland and the surrounding area.
• Able to comfortably and safely perform duties in private residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct approximately 45-minute in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a person’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships with people and help connect them with care suited to their needs.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Work as a clinician colleague and fulfill the requirements of the specified role.
