About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to conduct in-home health evaluations and help bridge gaps in care. Physicians engage with people in their homes, build trusted relationships, and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include time for one-on-one conversation, answering health questions, reviewing medication and medical history, and conducting a physical evaluation. Additional diagnostic tests may be provided if ordered by the person’s health plan. Physicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role covers Portland and surrounding areas, with local and in-state travel as needed, and may include virtual visits.

Requirements

• Active, unrestricted medical license(s) in the coverage area(s).
• Ability to cover Portland and the surrounding area.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer health questions and build trusted relationships with people during visits.
• Provide additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
