About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations for health plan members. The purpose of the visits is to build trusted relationships, help identify gaps in care, and connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and a clinical workflow and are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role may also include virtual visits and providing diagnostic and preventive services.

The primary workplace is members’ private homes. Home conditions vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these environments. Local and in-state travel is required as needed. Part-time employees receive select benefits, malpractice insurance coverage during Signify Health-related activities, supplies, and other perks.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan.
• Build trusted relationships with members and help connect them to appropriate care.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Travel locally and within licensed states as needed.
• Fulfill the requirements of the specified clinician role.
