About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations for health plan members in the Seattle/King coverage area. Physicians engage with members in their homes, build trusted relationships, answer health questions, and help bridge gaps in care. Visits take about 45 minutes and include a medication and medical history review and a physical evaluation. Depending on the member’s health plan, visits may also include additional diagnostic tests. Physicians use an iPad and an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. Virtual visits may be conducted as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Local and in-state travel is required as needed.

Requirements

• Active, unrestricted physician license(s) in the Seattle/King coverage area.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and help connect them with appropriate care.
• Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Fulfill the requirements of the specified clinician role and contribute to efforts to improve health outcomes.
