About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in the Seattle/King County area. Clinicians engage with people in their homes, build trusted relationships, help bridge gaps in care, and provide flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the person's health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person's current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role may also involve virtual visits and local travel within licensed states as needed.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct In-Home Health Evaluations and build trusted relationships with health plan members.
• Review members' medication and medical history and perform a physical evaluation.
• Perform additional diagnostic and preventive services when indicated, such as spirometry and diabetic eye exams.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated and fulfill the requirements of the clinician role.
