About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide In-Home Health Evaluations to health plan members in their homes. The role focuses on building trusted relationships, improving health outcomes, and helping bridge gaps in care through flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and a clinical workflow and are not required to prescribe medication, order laboratory tests, or change a person’s current treatment regimen. The primary workplace is members’ private residences, which may include household pets, secondhand smoke, and other airborne allergens or irritants. The position may also involve local travel, travel within licensed states as needed, and virtual visits. Full-time employees are eligible for a comprehensive benefits package.

Requirements

Active, unrestricted medical license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Conduct one-on-one In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
Provide additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Use an iPad and the provided clinical workflow to complete evaluations.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide all Signify Health services indicated for the role.
Help connect members with appropriate care and support efforts to improve health outcomes.
