About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations in Manhattan, New York. The role involves helping bridge gaps in care and building trusted relationships with health plan members through personalized evaluations in their homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The position may also involve virtual visits and local travel within licensed states. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Requirements

Active, unrestricted physician license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in members’ homes, including environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Provide additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Use an iPad and the clinical workflow for each evaluation.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Help connect people with appropriate care and support improved health outcomes.
Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
