In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Kennewick, WA
About The Position
Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians engage with health plan members in their homes, build trusted relationships, and connect members with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform duties safely and comfortably in these conditions. Local travel and travel within licensed states may be needed, as may virtual visits. The role is based around Kennewick and Benton County, Washington, and is scheduled for 40 hours per week.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic and preventive services when indicated, such as spirometry and diabetic eye exams.
- Build trusted relationships with health plan members and help connect them to appropriate care.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
