About The Position

Signify Health, part of CVS Health, is seeking a per diem clinician to provide in-home health evaluations to health plan members in Spokane, Washington. Clinicians meet with members in their homes to build trusted relationships, answer health questions, and help bridge gaps in care. Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a member’s current treatment regimen.

The role includes local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, which can have varied conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments.

This is a part-time, per diem position with an anticipated 12 weekly hours. Compensation is an hourly base rate plus an additional incentive structure, which may include incentives for completed visits. The posted pay range is $46.03–$99.14 per hour. Per diem employees receive malpractice insurance coverage for Signify Health-related activities, supplies, and other perks. Depending on hours worked and eligibility, benefits may include dental and vision coverage, wellness resources, employee discounts, access to certain voluntary benefits, and other programs.

Requirements

• Active, unrestricted license(s) in the coverage area(s), including Spokane, Washington.
• Board certification.

Responsibilities

• Conduct in-home health evaluations, typically lasting about 45 minutes.
• Review members’ medications and medical histories and perform a physical evaluation.
• Perform additional diagnostic and preventive services when indicated, such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Engage with members, answer health questions, and help connect them with care suited to their needs.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated.
