In-Home Nurse Practitioner or Physician Assistant Per Diem - Johnstown
About The Position
Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations. Clinicians engage with health plan members in their homes, build trusted relationships, and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The role involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. This is a part-time, per diem position with an anticipated 12 weekly hours. Compensation includes an hourly base rate and may include additional incentives, such as per-visit payments. The posted base pay range is $41.85–$90.13 per hour. The position is eligible for benefits and programs, subject to hours worked and eligibility.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.
- Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Build trusted relationships with members and help connect them to care suited to their needs.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Use an iPad and the clinical workflow for each evaluation.
- Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
