About The Position

Signify Health, part of CVS Health, is seeking a per diem Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members. Clinicians engage with members in their homes, build trusted relationships, and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and a clinical workflow and are not required to prescribe medication, order lab tests, or change members’ current treatment regimens.

The primary workplace is members’ private residences, which may contain pets, secondhand smoke, or other airborne allergens or irritants. The role may also include virtual visits and local travel, including travel within licensed states as needed. Per diem employees receive an hourly base rate plus potential incentives, malpractice insurance coverage during Signify Health-related activities, and supplies and other perks.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screenings.
• Build trusted relationships with members and help connect them with appropriate care.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
