About The Position

Signify Health, part of CVS Health, is seeking a per diem clinician to provide in-home health evaluations and help bridge gaps in care. Clinicians conduct approximately 45-minute visits in health plan members’ homes, connecting one-on-one, answering health questions, reviewing medications and medical history, and performing a physical evaluation. Additional diagnostic tests, such as diabetic eye exams or spirometry, may be included when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medication, ordering lab tests, or changing a member’s current treatment regimen.

The clinician is part of a national network focused on improving health outcomes and connecting people with appropriate care. The role requires local travel and travel within licensed states as needed, as well as virtual visits when needed. Primary coverage locations include Denver and Douglas County, Colorado.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Per diem compensation includes an hourly base rate plus an additional incentive structure, such as incentives per completed visit. Malpractice insurance coverage applies during Signify Health-related activities. Supplies and other perks are provided.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and provide time for one-on-one engagement during visits.
• Perform additional diagnostic and preventive services, such as spirometry and diabetic eye exams, when indicated by the health plan.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated.
