In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Yakima, WA
About The Position
Signify Health, part of CVS Health, is hiring a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in Yakima, Washington. Clinicians engage members in their homes, build trusted relationships, help bridge gaps in care, and connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role requires local travel and travel within licensed states as needed. The position is full time, with 40 anticipated weekly hours.
Requirements
- Active, unrestricted license(s) in the coverage area(s), including Yakima.
- Board certification.
- Ability to safely and comfortably perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations and engage members in their homes.
- Review members’ medications and medical histories and perform physical evaluations.
- Provide additional diagnostic tests when ordered by the member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Help members understand their health, answer questions, and connect them with appropriate care.
