About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide home healthcare to patients of all ages in Ridgecrest and surrounding areas. This is an independent contractor position with a flexible schedule, no minimum patient requirement, and per-visit compensation. Clinicians choose their preferred coverage areas and decide how many patients to treat.

The clinician determines each patient’s plan of care and observes, records, and reports responses to treatment and changes in condition. FeldCare Connects provides administrative support with assignments, communication, scheduling, care coordination, and quality assurance. Clinicians use the Cliniconnects app to accept or decline referrals and an electronic medical record system for documentation.

FeldCare Connects is an app-based referral network connecting clinicians with home health agencies and hospice providers to help homebound patients access care in their communities.

Requirements

• Current, active, and unrestricted Physical Therapist license issued by a state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Ability and enthusiasm to work with patients of all ages.

Responsibilities

• Provide home healthcare to patients of all ages.
• Determine the plan of care for each patient.
