About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide home healthcare to patients of all ages in Los Atos and surrounding areas. This independent-contractor role offers clinicians control over their schedules and coverage areas, with no minimum patient requirement. Compensation is based on per-visit rates and the number of patients treated.

FeldCare Connects provides administrative support with assignments, communication, scheduling, care coordination, and quality assurance. Clinicians provide one-on-one treatment, observe and document patients’ responses and changes in condition, and determine the plan of care. Documentation is completed through the Cliniconnects electronic medical record system, and clinicians can accept or decline referrals through the app.

FeldCare Connects connects clinicians with patients through home health agencies and hospices in California, Arizona, Texas, and Florida. The network aims to help homebound patients access care while reducing administrative work for agencies and clinicians.

Requirements

• Current, active, and unrestricted Physical Therapist license issued by the state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Ability and enthusiasm to work with patients of all ages.

Responsibilities
