About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide quality home healthcare to patients of all ages in Dinuba and surrounding areas. This is an independent contractor opportunity with flexible scheduling and coverage areas, no minimum patient requirement, and per-visit compensation. Contractors determine how many patients they treat and how much time to take off.

FeldCare Connects provides administrative support with assignments, communication, scheduling, care coordination, and quality assurance. Physical Therapists provide one-on-one treatment, observe and document patients’ responses and changes in condition, and determine the plan of care. Documentation is completed through the company’s intuitive electronic medical record system. Clinicians can use the Cliniconnects app to accept or decline patient referrals.

FeldCare Connects contracts with home health agencies and hospices to connect clinicians with patients in their communities. Its network includes over 2,000 clinicians and more than 900 home health agencies across California, Arizona, Texas, and Florida.

Requirements

• Hold a current, active, and unrestricted Physical Therapy license issued by a state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Complete an accredited Physical Therapist program.
• Have reliable personal transportation.
• Have a smartphone.
• Ability and enthusiasm to work with patients of all ages.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Provide one-on-one treatment and determine the plan of care.
