About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide home healthcare to patients of all ages in Galt and surrounding areas. This is an independent contractor position within an app-based referral network. Clinicians control their schedules and coverage areas, with no minimum patient requirement, and can choose how many patients to treat. FeldCare Connects provides administrative support for assignments, communication, scheduling, care coordination, and quality assurance. Clinicians deliver one-on-one treatment, determine the plan of care, and observe, record, and report patients’ responses to treatment and changes in their conditions. Documentation is completed through the Cliniconnects electronic medical record system, and clinicians can accept or decline patient referrals through the app.

FeldCare Connects connects clinicians with patients through contracts with home health agencies and hospices. The network includes more than 2,000 clinicians and works with over 900 home health agencies across California, Arizona, Texas, and Florida.

Requirements

• Ability and enthusiasm to work with patients of all ages.
• Current, active, and unrestricted Physical Therapist license issued by the state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Provide home healthcare to patients of all ages.
• Determine the patient's plan of care.
