About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide quality home healthcare to patients of all ages in Bear Valley Springs and surrounding areas. This is an independent contractor position with flexible scheduling and coverage areas and no minimum patient requirement. Contractors choose how many patients to treat and determine their earnings based on per-visit rates.

The role involves providing one-on-one treatment, determining the plan of care, and observing, recording, and reporting patients’ responses to treatment and changes in their conditions. FeldCare Connects provides administrative support with assignments, communication, scheduling, care coordination, and quality assurance. Clinicians use the Cliniconnects app to accept or decline referrals and an electronic medical record system for documentation.

FeldCare Connects contracts with home health agencies and hospices to connect clinicians with patients in their communities. The network includes more than 2,000 clinicians and contracts with more than 900 home health agencies across California, Arizona, Texas, and Florida.

Requirements

• Ability and enthusiasm to work with patients of all ages.
• Current, active, and unrestricted Physical Therapy license issued by the state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapist program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Provide quality home healthcare and one-on-one physical therapy treatment to patients of all ages.
