About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide quality home healthcare to patients of all ages in Kelseyville and surrounding areas. This is an independent contractor position. Contractors control their schedules and preferred coverage areas, with no minimum patient requirement, and are paid per visit based on the number of patients they choose to treat.

FeldCare Connects provides administrative support for assignments, communication, scheduling, care coordination, and quality assurance. Physical Therapists deliver one-on-one treatment, determine the plan of care, and observe, record, and report patient responses to treatment and changes in condition. Clinicians use the company's electronic medical record system and the Cliniconnects app to review and accept or decline patient referrals.

FeldCare Connects connects clinicians with patients through home health agencies and hospices, helping patients access care in their communities.

Requirements

• Ability and enthusiasm to work with patients of all ages.
• Current, active, unrestricted Physical Therapy license issued by a state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapist program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
