About The Position

FeldCare Connects is seeking a self-motivated Physical Therapist to provide home health care to adults of all ages in the Santa Ana area. The role involves assessing and evaluating patients’ care needs to promote mobility, reduce pain, restore function, and prevent disability. Based on these evaluations, the Physical Therapist determines and carries out treatment plans, uses interventions to improve patient well-being, and monitors progress. This is an independent contractor position. Clinicians in the network can make their own schedules and choose their preferred service area, receive administrative support with assignments, communication, scheduling, care coordination, and quality assurance, and work independently. Scheduling and documentation may be completed from home, but patient visits take place in patients’ homes, so the position is not remote.
