About The Position
This is a travel assignment for a Physical Therapist in a skilled nursing facility in Shepherdstown, West Virginia. The contract is for 17 weeks, starting on 09/14/2026. The position offers guaranteed hours and dedicated support for travel assignments.
Requirements
- Active, unrestricted physical therapy license to practice in West Virginia or eligible for WV licensure
- Doctor of Physical Therapy (DPT) degree from an accredited program
- CPR/BLS certification
- Experience in a skilled nursing facility or related rehabilitation setting preferred
- Strong documentation and communication skills; ability to work within an interdisciplinary team
- Ability to work the scheduled shift and commit to a 17-week travel assignment
Responsibilities
- Conduct PT evaluations for residents and develop individualized treatment plans to improve mobility and function
- Deliver therapeutic interventions including gait, balance, strength, and functional training
- Apply appropriate modalities and monitor patient response to treatment
- Document progress and care plans in the medical record; communicate with the care team
- Educate residents and families on home exercise programs, safety, and fall-prevention strategies
- Collaborate with an interdisciplinary team to support discharge planning and optimize functional outcomes
