About The Position

Physical Therapist needed for a travel assignment in a Skilled Nursing Facility setting in Fair Lawn, New Jersey. The position begins on 10/19/2026 for 13 weeks, with a weekly pay range of $2,451–$2,564 and guaranteed hours of 35 per week. Shifts run from 08:00 to 16:30, five days per week. Staffing is provided through Healthforce Healthcare LLC.

Requirements

Active physical therapy license in New Jersey (or eligibility for licensure)
Doctor of Physical Therapy (DPT) degree from an accredited program
Experience in physical therapy, with preference for skilled nursing facility or geriatric rehabilitation settings
Ability to work in a skilled nursing facility environment and to travel for the assignment
Strong communication and documentation skills; comfortable using electronic medical records

Responsibilities

Conduct comprehensive physical therapy evaluations for residents and develop individualized treatment plans to address mobility, strength, balance, and functional independence.
Implement evidence-based PT interventions and modalities appropriate for geriatric populations in a SNF setting.
Reassess patient progress regularly, document outcomes, and adjust goals and treatment plans accordingly.
Maintain accurate, timely documentation of evaluations, progress notes, discharge plans, and other required records in the clinical information system.
Collaborate with physicians, nursing staff, and the broader rehab team to coordinate care and optimize patient outcomes.
Educate residents and families/caregivers on home exercise programs, safety, fall prevention, and progression of activities of daily living.
