About The Position

This is a travel assignment for a Physical Therapist in a Skilled Nursing Facility located in Waldorf, Maryland. The assignment is for 13 weeks, with a start date of 09/28/2026. The role involves evaluating residents, developing and implementing individualized treatment plans, documenting progress, collaborating with an interdisciplinary team, educating residents and families, and ensuring compliance with professional standards.

Requirements

• Active Physical Therapist license in the United States with eligibility to practice in Maryland.
• Experience or demonstrated interest in skilled nursing facility or geriatric rehab settings.
• Ability to meet guaranteed hours of 38 per week during the assignment.
• Strong communication and documentation skills; ability to work within an interdisciplinary team.
• Facility-based, patient-centered approach with reliability and professionalism.

Responsibilities

• Evaluate residents’ mobility, balance, strength, and functional abilities to develop individualized PT plans of care.
• Implement and adjust treatment plans, including therapeutic exercises, manual therapy, and equipment use, to maximize functional independence.
• Reassess progress regularly, document outcomes, and update goals in the patient record.
• Collaborate with an interdisciplinary team (nursing, OT, SLP, and case management) to optimize rehabilitation outcomes.
• Educate residents and families on home exercise programs, safety strategies, and caregiver training to support continuation of care post-discharge.
• Ensure compliance with professional standards, facility policies, and infection control protocols.
