About The Position

This is a 13-week travel assignment for a Physical Therapist in a skilled nursing facility located in Puyallup, Washington. The role involves providing direct patient care, developing and implementing treatment plans, and collaborating with an interdisciplinary team. The assignment is in a long-term care / nursing home setting and guarantees 35 hours per week with a shift of 5x8 days.

Requirements

• Active Physical Therapist license in Washington state
• Ability to work effectively in a skilled nursing facility environment
• Strong documentation, communication, and clinical reasoning skills
• Availability to start on the specified date for a 13-week travel assignment

Nice To Haves

• Prior experience in geriatric rehabilitation or SNF settings preferred

Responsibilities

• Perform initial evaluations and ongoing assessments of residents to determine physical therapy needs.
• Develop, implement, and monitor individualized treatment plans aimed at improving mobility, strength, balance, gait, and functional independence.
• Provide direct patient care through therapeutic exercises, modalities, and restorative interventions, while ensuring safety and infection control protocols.
• Document patient progress, update treatment plans, and liaise with the facility’s interdisciplinary team to support discharge planning and care transitions.
• Educate residents and families on home programs, fall prevention, posture, and activity modification; ensure clear and accurate clinical notes.
