About The Position

Requirements

Active Physical Therapist license in New Mexico or eligibility to practice in New Mexico
Ability to work in a skilled nursing facility setting with geriatric rehab needs
Availability to commit to a 14-week travel assignment with 35 guaranteed hours per week
Strong clinical assessment, documentation, and communication skills for collaboration with an interdisciplinary team

Nice To Haves

Prior SNF or geriatric rehabilitation experience preferred

Responsibilities

Perform initial evaluations and ongoing reassessments of residents’ physical abilities to identify functional goals and develop an individualized treatment plan.
Implement evidence-based PT interventions focusing on gait, balance, strength, ROM, functional mobility, and activities of daily living.
Document treatment sessions clearly in the resident’s medical record and communicate progress to the interdisciplinary team.
Monitor safety, apply appropriate modalities and therapeutic exercises, and adapt plans based on patient response and goals.
Educate residents and families on home exercise programs, fall prevention, and transfer/ambulation strategies to support continuity of care after discharge.
