Requirements

Ability to work effectively in a skilled nursing facility environment and with an interdisciplinary team.

Responsibilities

Conduct initial and ongoing physical therapy evaluations for residents in a skilled nursing setting.
Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
Administer therapy sessions (e.g., therapeutic exercises, gait training, and modalities) and monitor patient progress.
Document assessments, goals, treatment plans, and progress notes in a timely and compliant manner.
Collaborate with an interdisciplinary team to support discharge planning and optimize resident outcomes.
Educate residents and families on home exercise programs, safety, and continuity of care.
