About The Position

This travel assignment places you in a skilled nursing setting, providing physical therapy services to residents. The position is a temporary travel assignment under Healthforce Healthcare LLC. An interprofessional team approach supports patient-focused care and planned discharges.

Requirements

Ability to work effectively in a Skilled Nursing Facility setting.
Strong clinical skills in geriatric rehabilitation and functional mobility.

Willingness to participate in a travel assignment and comply with facility policies.

Responsibilities

Perform PT evaluations and develop individualized treatment plans for geriatric residents.
Deliver skilled physical therapy interventions including therapeutic exercises, gait training, balance activities, edema management, and functional mobility training.
Monitor patient progress, document outcomes, and adjust treatment plans to meet goals.
Collaborate with interdisciplinary team (nursing, therapy, physicians, social work) to support rehabilitation and discharge planning.
Ensure safety, infection control, and adherence to regulatory standards in treatment sessions and clinical documentation.
