About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide home healthcare to patients of all ages in Morgan Hill and surrounding areas. The position is for an independent contractor, with flexible scheduling and coverage areas and no minimum patient requirement. Clinicians choose how many patients to treat and are compensated at per-visit rates.

The Physical Therapist provides one-on-one treatment, determines the plan of care, and observes, records, and reports patients’ responses to treatment and changes in their conditions. FeldCare Connects provides administrative support with assignments, communication, scheduling, care coordination, and quality assurance. Clinicians use the Cliniconnects app to accept or decline referrals and an electronic medical record system for documentation.

FeldCare Connects contracts with home health agencies and hospices to connect clinicians with patients in their communities.

Requirements

• Current, active, unrestricted Physical Therapy license issued by the state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapist program.
• Ability and enthusiasm to work with patients of all ages.
• Reliable personal transportation.
• Smartphone.
