About The Position

This is a travel temporary assignment for a Physical Therapist in a long-term care setting, specifically a skilled nursing facility in Moses Lake, Washington. Requirements

Active Physical Therapist license eligible for practice in Washington state or already licensed.
Doctor of Physical Therapy (DPT) degree or equivalent required.
Current Basic Life Support (BLS) certification.
Ability to work effectively in a SNF environment with a focus on functional outcomes and resident safety.
Strong communication, documentation, and collaboration skills; comfortable with a temporary, split-placement assignment.

Nice To Haves

1+ year of physical therapy experience in a skilled nursing facility or similar long-term care setting preferred.

Responsibilities

Perform comprehensive PT evaluations to determine rehabilitation needs and create individualized treatment plans for residents in a skilled nursing setting.
Deliver physical therapy interventions (strengthening, mobility, gait training, balance, ROM) and adjust plans based on progress and goals.
Document evaluation findings, treatment plans, daily progress notes, and outcomes in resident records following policy and coding guidelines.
Monitor safety, prevent falls, and manage assistive devices, equipment, and adaptive strategies as needed.
Collaborate with nursing, medicine, therapy colleagues, and families to coordinate care, discharge planning, and transitions of care.
