About The Position

This is a travel temporary assignment for a Physical Therapist in a long-term care setting, specifically a skilled nursing facility in Moses Lake, Washington. Requirements

  • Active Physical Therapist license eligible for practice in Washington state or already licensed.
  • Doctor of Physical Therapy (DPT) degree or equivalent required.
  • Current Basic Life Support (BLS) certification.
  • Ability to work effectively in a SNF environment with a focus on functional outcomes and resident safety.
  • Strong communication, documentation, and collaboration skills; comfortable with a temporary, split-placement assignment.

Nice To Haves

  • 1+ year of physical therapy experience in a skilled nursing facility or similar long-term care setting preferred.

Responsibilities

  • Perform comprehensive PT evaluations to determine rehabilitation needs and create individualized treatment plans for residents in a skilled nursing setting.
  • Deliver physical therapy interventions (strengthening, mobility, gait training, balance, ROM) and adjust plans based on progress and goals.
  • Document evaluation findings, treatment plans, daily progress notes, and outcomes in resident records following policy and coding guidelines.
  • Monitor safety, prevent falls, and manage assistive devices, equipment, and adaptive strategies as needed.
  • Collaborate with nursing, medicine, therapy colleagues, and families to coordinate care, discharge planning, and transitions of care.
