About The Position

This is a travel Physical Therapist position for a skilled nursing facility assignment in Skowhegan, Maine. The contract is for 13 weeks, with a start date of 08/31/2026. Requirements

Doctor of Physical Therapy (DPT) degree from an accredited program.
Active Physical Therapist license in Maine or eligibility for licensure in Maine.
Ability to work in a skilled nursing facility setting and deliver care on a travel assignment.
Willingness to travel for a 13-week assignment and adhere to assignment schedule (08:00–16:30, 5 days per week) with 35 guaranteed hours per week.
Strong communication and teamwork skills; comfortable documenting care and collaborating with an interdisciplinary team.

Nice To Haves

Relevant experience in geriatric rehabilitation or SNF settings preferred.

Responsibilities

Conduct comprehensive PT evaluations for residents and develop individualized treatment plans targeting mobility, strength, balance, gait, and range of motion.
Implement evidence-based interventions to improve functional independence and safety with activities of daily living.
Monitor progress, modify plans as needed, and document outcomes in the resident’s medical record.
Provide patient and family education on exercises, safety strategies, and home programs; collaborate with the interdisciplinary team to coordinate care.
Participate in discharge planning, referrals, and transitions of care to support continued progress.
