Requirements

Active Physical Therapist license or eligibility to practice in Massachusetts.
Ability to work in a Skilled Nursing Facility setting and manage a patient caseload.

Willingness to participate in a 40-hour weekly schedule.
Eligibility to work in the United States.

Nice To Haves

Experience in SNF or geriatric rehabilitation is desirable but not required.

Responsibilities

Assess residents’ physical abilities and develop individualized plans to improve mobility, strength, balance, and daily function.
Implement therapy interventions, including gait training, transfers, endurance activities, and pain management.
Reassess progress, document outcomes, and adjust plans of care in collaboration with the interdisciplinary team.
Educate residents and families on home programs, safety, activity modification, and self-management strategies.
Ensure accurate documentation, timely progress notes, and compliance with regulatory standards.
Promote safety during therapy sessions and coordinate with the care team to optimize outcomes.
