Requirements

Active physical therapy license in good standing in the practice state or eligibility for licensure transfer per assignment policies.
Strong clinical assessment, documentation, and communication skills.
Ability to work on-site in a skilled nursing facility setting.
CPR/BLS certification.
Ability to travel and commit to a 13-week assignment.
Flexible, team-oriented approach and reliable attendance.

Responsibilities

Perform comprehensive PT evaluations and develop a plan of care for SNF residents.
Implement evidence-based interventions: therapeutic exercise, gait training, balance, ROM, and pain management.
Reassess progress regularly, adjust treatment plans, and document outcomes in resident records.
Collaborate with nursing and rehab teams; contribute to discharge planning and caregiver education.
Ensure patient safety, maintain HIPAA-compliant documentation, and comply with facility policies and regulations.
