Requirements

Active Physical Therapist license in New Hampshire or eligible for licensure
Doctor of Physical Therapy (DPT) degree
Current CPR/BLS certification or ability to obtain
Ability to work effectively in a Skilled Nursing Facility setting
Relevant clinical experience in physical therapy
Eligibility to work in the United States

Geriatrics/SNF experience preferred

Responsibilities

Perform initial evaluations and ongoing re-assessments of residents to determine PT needs
Develop and implement individualized treatment plans to improve mobility, strength, balance, and function
Lead therapeutic exercises, gait training, manual therapy, and ROM activities as appropriate
Monitor patient progress, adjust plans of care, and document outcomes in the chart
Collaborate with the interdisciplinary team to coordinate care and discharge planning
Educate residents and families on home exercise programs, safety, and fall prevention
Ensure patient safety, infection control, and adherence to regulatory standards during therapy sessions
