About The Position
Physical Therapist sought for a travel assignment in a skilled nursing facility located in Laconia, New Hampshire. Requirements
- Active Physical Therapist license to practice in New Hampshire or eligibility to obtain licensure
- Graduation from an accredited physical therapy program (DPT or equivalent)
- Prior clinical PT experience; skilled nursing facility/geriatric setting preferred
- Ability to commit to a 13-week travel assignment and 35 hours per week
- Strong documentation and communication skills; comfortable working in a team environment

Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents
- Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence
- Administer therapy interventions and monitor progress toward goals
- Reassess and adjust plans based on resident response; document progress notes, goals, and treatment plans accurately
- Collaborate with the interdisciplinary care team to coordinate care, discharge planning, and family education
- Ensure safety, infection control, and compliance with regulatory and facility policies
