Requirements

Doctor of Physical Therapy (DPT) or PT degree from an accredited program
PT licensure: active license or eligibility to obtain licensure as a Physical Therapist per state regulatory requirements
1+ year of physical therapy experience; outpatient experience preferred
Ability to work effectively in an outpatient hospital-based rehabilitation setting
Strong communication and teamwork skills
EPIC experience preferred

Nice To Haves

1+ year of physical therapy experience preferred
EPIC experience preferred

Responsibilities

Conduct 1-hour patient evaluations and 45-minute daily treatment sessions
Develop and implement individualized plans of care for orthopedic, sports, and work-related injuries
Collaborate with a multidisciplinary rehabilitation team to coordinate patient care
Document care in EPIC
Manage referrals from orthopedic surgeons, family practice providers, and the acute care team
Monitor patient progress and adapt treatment plans to achieve optimal outcomes
