About The Position

Responsibilities

Conduct inpatient acute physical therapy evaluations and develop individualized treatment plans.
Provide and adjust therapeutic interventions to improve mobility, strength, balance, and discharge readiness.
Monitor patient progress, document outcomes, and update care plans in accordance with hospital standards.
Collaborate with physicians, nurses, and the broader rehabilitation team to coordinate care and discharge planning.
Educate patients and families on home exercise programs, safety, and post-discharge care.
