About The Position

This is a travel assignment for a Physical Therapist in a home healthcare - hospice setting in Oakland, CA. The contract is for 13 weeks, with a start date of 08/27/2026. The position guarantees 40 hours per week during the day shift, with a weekly pay range of $2,802–$3,047.

Requirements

• Active California Physical Therapist license
• At least 1 year of Home Health experience
• Ability to perform Start of Care visits and associated documentation
• Proficiency with the Kinnser documentation system or willingness to learn
• Must be able to work in a home health setting and support territory needs as assigned

Responsibilities

• Prioritize start of care visits and manage scheduling to ensure timely initiation of home health services
• Provide clinical supervision for Physical Therapy Assistants as assigned
• Complete start of care visits with next-day reporting
• Document clinical notes and other required documentation within 48 hours of each visit
• Maintain timely, professional communication with supervisor, intake, and office; respond during normal business hours
• Perform patient care within a home health framework and utilize the documented system (Kinnser) for ongoing records
