About The Position

This is a travel temporary assignment for a Physical Therapist in a long-term care (LTC) setting, specifically a nursing home, in Glendive, MT. The contract duration is 13 weeks, with a start date of 09/14/2026. The role involves performing evaluations, developing treatment plans, delivering evidence-based therapies, monitoring patient progress, ensuring safety, educating patients and families, and collaborating with the interdisciplinary team.

Requirements

Active Physical Therapist license in Montana or eligibility for MT licensure; in good standing
Doctor of Physical Therapy (DPT) degree or equivalent
Ability to work effectively in a long-term care setting with older adults
Experience in physical therapy; long-term care setting preferred
Strong communication and teamwork skills; able to document clearly and coordinate with care staff

Responsibilities

Perform initial PT evaluations for residents in the long-term care setting and develop individualized treatment plans in collaboration with the interdisciplinary team.
Deliver evidence-based therapies addressing mobility, gait and balance, strength, range of motion, and activities of daily living to restore or maintain function.
Monitor patient progress, document outcomes in the medical record, and adjust treatment plans as clinically indicated.
Ensure patient and staff safety during therapy sessions, adhere to infection control standards, and follow facility policies.
Educate patients and families on home exercise programs and activity modifications to support ongoing independence.
Collaborate with physical therapy aides and other disciplines to optimize rehabilitation outcomes and coordinate care transitions.
