About The Position

This is a 13-week travel assignment for a Physical Therapist in a Skilled Nursing Facility in Waterville, Maine. The role involves assessing and treating residents, developing individualized treatment plans, delivering PT interventions, monitoring progress, documenting care, and collaborating with the healthcare team. The position starts on 11/02/2026, with guaranteed 35 hours per week and a weekly pay range of $2,352–$2,558.

Requirements

  • Active Physical Therapist license in Maine or eligible for licensure by endorsement
  • Doctor of Physical Therapy (DPT) or equivalent degree
  • Minimum 1 year of PT experience in a skilled nursing facility or geriatric rehab setting
  • Ability to work in a skilled nursing facility environment and meet assignment requirements
  • Ability to obtain and maintain required credentialing for travel assignments; pass applicable background checks and screenings
  • Must be legally eligible to work in the United States

Responsibilities

  • Assess and evaluate residents' physical therapy needs; perform initial and ongoing evaluations; develop individualized treatment plans with measurable functional goals; coordinate with the care team to align with discharge planning.
  • Deliver PT interventions including gait training, balance training, strengthening, therapeutic exercises, ROM, and functional mobility tasks; utilize appropriate assistive devices and adaptive equipment.
  • Monitor patient progress using standard measures; adjust treatment plans as needed; document progress and communicate changes to the supervising PT and interdisciplinary team.
  • Document comprehensively in the electronic medical record; ensure compliance with HIPAA, regulatory standards, and facility policies; prepare discharge instructions and home exercise programs.
  • Collaborate with physicians, nursing staff, and other therapists (OT, SLP, aides) to optimize outcomes; participate in care planning meetings and educate patients and families.
  • Ensure safety, infection control, and adherence to facility policies; promote patient and family education on exercises, activity strategies, and fall prevention.
