About The Position
Concierge Home Care is hiring a Home Health Physical Therapist to provide one-to-one patient care in patients’ homes in Port St. Lucie County, Florida. The role offers a flexible, self-managed schedule, clustered territories designed to minimize drive time, consistent patient volume, and pay-per-visit compensation with Medicare differential pay and additional earning incentives. Average full-time earnings are listed as $120,000 or more. The therapist evaluates patients, develops individualized treatment plans, provides therapeutic exercise and mobility training, and educates patients and caregivers on safety and recovery. The role also involves collaboration with physicians and interdisciplinary teams, supervision of Physical Therapist Assistants, and completion of clinical documentation using streamlined technology. Concierge Home Care provides onboarding, mentorship, preceptorship, ongoing training, and clinical support. Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Requirements
- Active, unencumbered Florida Physical Therapist license.
- Valid driver’s license, reliable transportation, and auto insurance.
- Successful completion of an AHCA Level 2 background screening is a condition of employment.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
