About The Position
Concierge Home Care is hiring a Home Health Physical Therapist to provide one-to-one patient care in patients’ homes in Clay County, Florida. The position offers a pay-per-visit compensation model, flexible scheduling, autonomy, and opportunities to increase earnings based on productivity. Territories are designed to minimize drive time through clustered scheduling, and consistent patient volume is provided. The therapist evaluates patients, develops individualized treatment plans, provides therapeutic exercises and mobility training, educates patients and caregivers on safety and recovery, collaborates with physicians and interdisciplinary teams, supervises Physical Therapist Assistants, and completes documentation using streamlined technology. The role includes structured onboarding, mentorship, preceptorship, ongoing training, and career advancement opportunities. Employment is contingent on successfully completing an AHCA Level 2 background screening.
Requirements
- Active, unencumbered Florida Physical Therapist license.
- Valid driver’s license, reliable transportation, and auto insurance.
- Successful completion of an AHCA Level 2 background screening is required for employment.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
