Occupational Therapist, OT, Flexible Weekday Schedule
About The Position
Concierge Home Care is hiring Home Health Occupational Therapists to provide one-to-one care in patients’ homes in Jacksonville, Florida. Therapists may choose a Northside or Southside coverage area, with territories designed to reduce drive time through clustered scheduling. The pay-per-visit compensation model offers $65–$75 or more per visit, Medicare differential pay, mileage reimbursement, and consistent patient volume. The position offers flexibility and autonomy: therapists create and manage their own schedules, with no weekends or holidays required (optional flexibility is available). The role does not involve hospital shifts or 12-hour rotations. Clinicians receive structured onboarding, mentorship and preceptorship with experienced clinicians, ongoing training and development, free CEUs, and career advancement opportunities. AI-assisted charting and the WellSky/Kinnser EMR are used to streamline documentation. Employment options include PRN or 1099.
Requirements
- Active, unencumbered Florida Occupational Therapist license.
- Valid driver’s license, reliable transportation, and auto insurance.
- CPR certification.
- Ability to supervise COTAs.

Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Perform home safety assessments and recommend modifications.
- Provide therapy to improve activities of daily living, functional mobility, and independence.
- Educate patients and caregivers on adaptive techniques and equipment.
- Collaborate with physicians and interdisciplinary teams.
- Supervise Certified Occupational Therapy Assistants (COTAs).
- Complete documentation using streamlined technology.
