About The Position
Concierge Home Care is hiring a Home Health Physical Therapist to provide one-to-one patient care in patients’ homes in Indian River County, Florida. The position offers a flexible schedule, autonomy, a pay-per-visit compensation model, Medicare differential pay, quarterly bonus opportunities, mileage reimbursement, a data plan stipend, and consistent patient volume. Territories are designed to minimize drive time through clustered scheduling. Average full-time earnings are stated as $120,000+. The therapist evaluates patients and develops individualized treatment plans, provides therapeutic exercise and mobility training, educates patients and caregivers on safety and recovery, collaborates with physicians and interdisciplinary teams, supervises Physical Therapist Assistants, and completes documentation using streamlined technology. The organization offers AI-assisted charting and a user-friendly WellSky/Kinnser EMR, along with structured onboarding, mentorship, preceptorship, ongoing training and development, free CEUs, and career advancement opportunities. Employment is contingent on successful completion of an AHCA Level 2 background screening.
Requirements
- Active, unencumbered Florida Physical Therapist license.
- Valid driver’s license, reliable transportation, and auto insurance.
- Successful completion of an AHCA Level 2 background screening is a condition of employment.
Responsibilities
