Physical Therapist (PT) – Flexible Schedule
About The Position
Concierge Home Care is hiring a Home Health Physical Therapist to provide one-to-one patient care in patients’ homes in Lakeland and Polk County, Florida. Territories are designed to minimize drive time through clustered scheduling. The position offers a pay-per-visit compensation model, schedule autonomy, and the opportunity to increase earnings based on productivity. The job posting reports pay of $70+ per visit plus Medicare differential pay, average full-time earnings of $120,000+, quarterly bonus opportunities, mileage reimbursement, and a data plan stipend. Consistent patient volume is provided. The therapist evaluates patients, develops individualized treatment plans, provides therapeutic exercises and mobility training, and educates patients and caregivers on safety and recovery. The role collaborates with physicians and interdisciplinary teams, supervises Physical Therapist Assistants, and completes documentation using streamlined technology, including AI-assisted charting and the WellSky/Kinnser EMR. The employer describes structured onboarding and mentorship, preceptorship with experienced clinicians, ongoing training and development, free CEUs, and career advancement opportunities. Health insurance starts after 30 days, and PTO begins accruing on the first day. Employment is contingent on successfully completing an AHCA Level 2 background screening.
Requirements
- Active, unencumbered Florida Physical Therapist license.
- Valid driver’s license, reliable transportation, and auto insurance.
- Successful completion of an AHCA Level 2 background screening is a condition of employment.
